（1）申请表；
（2）外商投资企业关于增加分销经营范围的一致通过的董事会决议（外资企业提交股东会决议或股东决定）；
（3）外商投资企业合同章程修改协议；
（4）外商投资企业的进出口商品目录；
（5）外商投资企业批准证书、营业执照复印件；
（6）外商投资企业原合同章程复印件；
（7）法定验资机构出具的注册资本已经缴足的验资报告。
